Thanks for posting this chat.
Didn’t get a chance to comment on the use of US for spinals. You know I cannot resist!
Personally I have not used US for the spine / epidural in anger. That may be because I do all mine in a heavily pregnant, oedematous women in the throes of labour – and hence the ED spinal is relatively civilised and “easy”.
However, if I had to do it – I would follow Dr Chris Mitchell’s advice – posted here on a nice presentation:

This is an evolving area of practice for US and Anaesthesia – watch out for evidence. Maybe one day it will be like doing IJ CVCs – only madmen do it blind!
