Hi folks. I spoke to Dr Chris Groombridge, EM physician , who recently published a study on common preoxygenation strategies. Dr Nicholas Chrimes provided commentary from an anaesthetic viewpoint and Dr Alan Garner of Careflight Sydney gave prehospital commentary from his service’s aspect. I then recorded with Dr Clare Hayes-Bradley, an anaesthetist and prehospital doctor with Greater Sydney area HEMS and who also published a study examining preoxygenation strategies as well.
Nick kindly provided this diagram below, describing the factors relevant to Safe Apnoea time and the benefits of nasal cannula oxygenation.
Take home messages: Use well fitted mask with BVM or Mapelson type breathing circuit. Add nasal cannula if you think the patient will require extra oxygen flow due to respiratory demand. Avoid Non Rebreather Mask if possible, as too much leak. If only choice then definitely add second oxygen source like nasal cannulae.
