I managed to implement a weekly emergency sim program in my previous Aussie ED – was well received. Only possible because it had support from above. Difficult getting a ‘whole team’ approach (consultants, reg, residents, nurses) together, often there were many more docs than nurses in one scenario, making for unbalanced teams and therefore unrealistic sim. However you need to start somewhere, build goodwill and show benefit of the sim (‘sim it and they shall come’).
